Joining Instructions

The candidates who are eligible to apply for admission must ensure submission of the following attested copies of documents along with their application form.

1. Recent Photograph (with blue background, 1×1 inch) : (Four Copies)
2. Recent Photograph (with blue background, Passport size) : (Twelve Copies)
3. Character Certificate : (One Copy)
4. SSC / O – Level : (Six Copies)
5. HSSC / A – Level : (Six Copies)
6. Equivalent Certificate form IBCC for O/A Level : (Six Copies)
7. Student’s NIC / B Form : (Six Copies)
8. Domicile : (Two Copies)
9. Original Submitted Fee Chalan copies
10. Parent’s / Guardian’s NIC : (Six Copies)
11. NUMS- MDCAT   Entry Test result /  : Original
12. NOC for other Board / University and Undertaking on: Original Stamp Paper submitted after the admission
13. Mental and Physical Fitness Certificate to be submitted: Original after the admission as per the requirement of PM&DC.
14. Wealth statement of either Parent/guardian
15. Income tax return of parents/guardian
16. Insurance certificate to ensure the payment of fees.
17. COVID-19 Test Report from authentic Laboratory.
18. “Application Form for Issue of Security Pass” duly completed and verified by SHO Local Police Station. (To be submitted at the time of admission positively).
